core.accounts allows several billing accounts per organization; signup creates one, which the billing model calls the default, but nothing marks it. Checkout, the reconcile on return from Checkout (internal/server/server.go) and the organization page's billing summary each take the oldest; the invoices page reads all. Each account has its own Stripe customer.

Done when: the payments model states one account per organization with a unique constraint, or a default recorded in a column with one query returning it, and no handler indexes into a list of accounts.
