What a person cannot do today

An operator who changes an org type's default plan, or reorders a ladder's tiers, cannot say why. Issuing a grant takes a note, shown with the grant in its history, and so does a batch of rule changes to an entitlement set. These two changes take none, although either can move every organization of a type.

What they should be able to do

Write a note on the default-change form and on the reorder form, and read it wherever that change's history is shown: on the Tier changes rows the change wrote, and on the change's own page if it gets one.

Why it matters

A change that moves many organizations at once is the one most worth explaining later. Without a note, the history records only that it happened.

Where

The org type's default-change form, the ladder's reorder form, and the Tier changes list.

Done when

Both forms offer a note. The note is stored on the change's act record (#187), and every Tier changes row the change wrote shows it. That needs those rows to point at their act (#179).
